The Veterans Administration is looking into allegations that the Van Zandt VA Medical Center failed to follow-up with brain injury patients.
 
The VA Office of Inspector General launched the investigation after Van Zandt employee, James DeNofrio, inquired about another allegation.
 
DeNofrio, the administrative officer for the physical rehab department, found that between 2007 and 2013, 400 patients did not receive the necessary care.
 
He says after he reported that to the local VA, a later investigation found that 90 patients including six who had died and one who committed suicide, were falsely listed as receiving care.
